Provides collaborative clinical pharmacy services with physicians and healthcare staff to deliver coordinated patient care. Manages patient populations using disease registries, reports, education, and medication optimization to improve therapeutic outcomes. Requires a pharmacy degree, PGY-1 residency or equivalent experience, Washington pharmacist licensure, board certification within two years, preceptor licensure when applicable, and relevant pharmacy practice experience.

Provides collaborative clinical pharmacy services with physicians and healthcare staff, delivering comprehensive patient care and managing anticoagulation therapy. Uses patient registries and clinical reports to identify target populations, provide disease and medication education, optimize drug therapy, and improve outcomes. Requires a pharmacy degree, PGY-1 residency or equivalent experience, Washington pharmacist licensure, board certification within two years, preceptor licensure when eligible, and relevant pharmacy practice experience.

Performs CT examinations according to physician orders, using CT equipment and adapting imaging techniques to patients’ individual and age-related needs. Produces optimal diagnostic images while applying clinical judgment and supporting patient-focused care. Requires Oregon medical imaging licensure, BLS certification, and national radiography or nuclear medicine registration, with CT certification required within one year.
