Teams at Prisma Community Care

Provides HIV-focused case management for Ryan White clients, including psychosocial assessments, individualized service planning, care coordination, insurance and patient-assistance navigation, referrals, advocacy, harm-reduction education, and ongoing monitoring. Maintains an active caseload, documents encounters and billable services, supports culturally responsive care for LGBTQIA+ and marginalized communities, and collaborates with internal teams and external agencies. Requires approximately 10% travel outside Phoenix and may include evening or weekend work.
