Coordinates Minimum Data Set assessments, resident care plans, and interdisciplinary team meetings for skilled nursing facility residents. Ensures compliance with federal, state, and facility requirements while collaborating with physicians, nursing staff, residents, families, and other departments. The role may include floor work and daily nursing duties, requiring strong communication, organization, prioritization, direction, and motivation skills.

Performs radiographs, CT scans, and/or mammography while providing collaborative patient imaging services. Handles standby, after-hours, weekend, holiday callbacks, and must arrive within 30 minutes. May also perform protocolling, scheduling, phone coverage, and insurance-related support. Newly graduated technologists may receive CT or mammography training, with required registry completion within one to two years.

Provides comprehensive perioperative nursing care before, during, and after surgical procedures. Assesses patients, develops and evaluates individualized care plans, promotes safety, transfers care appropriately, and supports surgical teams. The nurse also assists with orienting new personnel and serves as a resource for patient-care issues. This per diem role requires on-call availability and is based at Plumas District Hospital in Quincy, California.

Coordinates patient care during clinic visits, rooms patients, records vital signs, performs laboratory work, prepares rooms and equipment, assists with procedures, updates patient medical records, and documents care accurately in the electronic medical record. Supports nursing personnel and medical staff while recognizing patient needs and initiating appropriate assistance or triage.

Provides clerical and reception support to the Director of Nursing, nursing staff, residents, visitors, vendors, and hospital departments. Responsibilities include greeting and directing visitors, screening and routing calls, maintaining files and calendars, scheduling meetings, coordinating resident care communications, preparing correspondence, and completing faxing, copying, and scanning. The role requires prioritizing multiple demands, supporting nurse-station workflow, maintaining confidentiality and professionalism, and following infection-control procedures.

Coordinates and maintains provider clinic schedules, processes schedule changes, updates Revenue Cycle and online scheduling systems, and communicates discrepancies to healthcare leaders. Serves as a process improvement subject matter expert by analyzing workflows, mapping processes, implementing controls, leading strategic initiatives, training staff, and applying Lean Six Sigma methodologies to improve operational performance, reduce costs, and support patient-centered care.

Coordinates social services for skilled nursing facility residents by assessing psychosocial needs, developing individualized care plans, completing MDS documentation, maintaining progress notes, arranging referrals, and coordinating appointments and departmental support. Conducts resident interviews and assessments while connecting residents with community resources and professional services.
