Manages home-health documentation and outstanding physician orders by coordinating with referral partners, clinicians, and internal departments. Maintains accurate physician and referral-source information in WellSky, tracks follow-ups, prepares and escalates orders, obtains signatures and missing paperwork, reviews reports and potential write-offs, and documents daily activity while complying with HIPAA requirements. The role requires regular field travel to referral offices and strong communication, organization, typing, and multitasking skills.

Lead clinical operations at a home health branch, ensuring quality, compliance, survey readiness, OASIS accuracy, and performance on HHVBP/HHCAHPS. Supervise clinical managers/field staff, run QAPI activities, provide mentoring and competency oversight, and partner with operations on staffing and productivity.
