Provide clinical supervision and leadership for a case management team serving formerly chronically homeless tenants. Oversee implementation of best practices (Motivational Interviewing, Trauma-Informed Care, Harm Reduction), manage daytime operations, coordinate with property management and community partners, respond to crises and participate in 24/7 on-call rotation, ensure documentation, data reporting, and support tenant engagement to maintain housing stability.

Provide intensive, on-site case management for tenants experiencing homelessness, mental illness, and substance use using Housing First, harm reduction, and motivational interviewing. Assist with benefits, healthcare access, employment, legal aid, daily living skills, individual service plans, crisis intervention, documentation, and collaboration with housing staff to promote housing stability.

Provide intensive, harm-reduction and trauma-informed case management to homeless individuals using Housing First. Maintain caseload, create service plans, assist with benefits, healthcare, education, employment, legal aid, daily living skills, documentation, crisis intervention, and collaborate with staff to promote tenant stability.
