Provide intensive, harm-reduction and trauma-informed case management to chronically homeless tenants. Maintain caseload, create service plans, assist with benefits, healthcare, employment, legal aid, ADL skills, documentation, and collaborate with housing team to stabilize tenants.

Provide intensive, trauma-informed case management using Housing First and Harm Reduction models for homeless individuals. Manage a caseload, assist with documentation, benefits, healthcare, employment, and daily living skills; create service plans, respond to crises, and collaborate with housing support staff to stabilize tenants.

Provide overnight on-site coverage and maintain property cleanliness at congregate housing for homeless and chronically homeless tenants. Respond to emergencies, promote social interaction and problem solving among tenants, represent the organization to the neighborhood, and comply with residency eligibility and municipal vetting requirements.
