Transport adolescents within the residential treatment facility safely and respectfully. Coordinate and respond to transport requests, assist and reassure patients during movement, maintain transport equipment and cleanliness, monitor and document patient condition, and report concerns to clinical staff.

Coordinate care for adolescents with behavioral and complex medical needs. Serve as primary contact for families, assess needs, create and adjust individualized care plans, monitor progress, advocate for patients, connect families with resources, provide education, and maintain confidential documentation per policies and regulations.

Provide behavioral health assessments, coordinate admissions and referrals, verify insurance, collaborate with multidisciplinary teams, maintain documentation, and deliver direct patient care (about 85% clinical time).
