Teams at Pair Team

Provide relationship-based case management for high-need Medicaid/Medicare members, performing telephonic and occasional in-person outreach, care coordination, appointment scheduling, referral management, and documentation using CRM and internal platforms while collaborating with multidisciplinary clinical teams.

Lead patient-focused product marketing: own patient research and segmentation, positioning and messaging, lifecycle and onboarding experiences, go-to-market for product and service launches, and measurement (A/B tests, funnel metrics). Bridge product and patient insight to inform roadmap, partner with growth, brand, clinical and operations, and ensure compliant, trust-preserving patient communications that improve activation, retention, and conversion.

Provide relationship-based case management for high-need Medicaid/Medicare members: outreach, engagement, care planning, referral coordination, support delegated clinical tasks, meet encounter metrics, and perform 2–3 monthly on-site community visits while primarily working remotely.
