Assists the Social Services Director in supporting nursing-home residents and families. Responsibilities include gathering social histories, monitoring behavior, coordinating transportation and care-plan meetings, maintaining resident documentation, supporting medical equipment and personal needs, reviewing care-plan compliance, reporting incidents, and following safety, privacy, and infection-control procedures.
