Provide in-home infusion care, administer and teach specialty pharmacy infusion therapies and injectables, access and maintain vascular access devices, perform assessments, wound care, medication management, and emergency procedures. Coordinate care with nursing leadership, document plan of care and patient changes, follow infection control, manage supplies, and travel within assigned region with on-call flexibility.

Provide quality in-home infusion care and teach self-administration of specialty pharmacy therapies and injectables. Perform vascular access (SPC/PIV/Midlines, PICC/ports), SCIG, phlebotomy, lab processing, wound care, assessments, medication management, and CPR/emergency procedures. Coordinate patient care with nursing leadership, maintain documentation and infection control, travel within assigned region, work flexible/alternate shifts, and complete required skills assessments and compliance checks.

Provide home health nursing for patients with tracheostomies and ventilators: perform assessments, develop/revise care plans, educate patients/families, collaborate with care teams, mentor staff, document and report condition changes, complete skills assessments, and follow regulatory and agency policies.
