Supervise daily clinical operations for a Santa Barbara home health team, oversee staff performance, ensure regulatory compliance, promote high-quality patient care, and support team development and quality improvement. The role requires an experienced California-licensed RN with supervisory experience and at least one year in home health or a related field.

Provides home health nursing care to patients in private homes, including ventilator and tracheostomy support. Responsibilities include developing care plans, educating patients and families, collaborating with care teams, monitoring and reporting condition changes, communicating with physicians, and mentoring coworkers.

Provides in-home and alternate-setting infusion care, including IVIG, SCIG, biologics, enzyme replacements, and injectable therapies. Teaches patients self-administration, accesses and maintains vascular access devices, performs phlebotomy, follows infection control procedures, coordinates care plans, monitors patient changes, and responds to emergencies. Requires travel within the assigned Rochester region, schedule flexibility, and occasional on-call field visits.
