Provides psychiatric and medical nursing care to patients in an Assertive Community Treatment program. Responsibilities include patient assessments, medication administration and management, treatment planning, clinical documentation, coordination with healthcare providers, medication organization and distribution, and participation in team meetings. The role operates onsite and in the community and requires collaboration with nurse practitioners, practical nurses, and treatment staff.

Provides psychiatric evaluations, medication reviews, diagnoses, treatment plans, and comprehensive mental health care for youth and adults. Conducts face-to-face assessments, maintains confidential clinical records, and provides guidance or training to clinical staff. The role is on-site for eight weekday hours per week and requires independent practice within New York licensure and prescribing requirements.

Provides administrative coordination for an Assertive Community Treatment program, managing referrals, intakes, discharges, medical records, billing reports, insurance eligibility, audits, scheduling, data tracking, budgets, and communication. Maintains provider contacts, office supplies, vehicle readiness, and program documentation. Supports participant and visitor safety, triages phone calls, coordinates psychiatric appointments, and accompanies staff in the community when safety concerns arise.
