Works with patients to address non-clinical barriers to care by connecting them to community resources (food, housing, transportation, financial assistance, insurance enrollment), assisting with forms, coordinating referrals, collaborating with providers and community partners, and maintaining knowledge of available resources.

Provide pediatric primary care as an ARNP or PA-C within an integrated community health center. Manage your own patient panel, collaborate with an interdisciplinary team, document care in Epic, participate in orientation and training, and support community-focused, whole-person care. Opportunity to engage in graduate medical education and qualify for loan repayment programs.

Provide patient-centered coordination for Office Based Addiction Treatment (OBAT): engage patients, conduct screenings, schedule appointments, assist with insurance, maintain EHR documentation, coordinate multidisciplinary care and community referrals, support program administration, outreach, data entry, and grant tracking while promoting harm reduction and recovery.
