Provide information, advocacy, and service coordination for individuals and families. Develop, monitor, and revise program plans; identify and coordinate services; complete required documentation; attend interdisciplinary meetings; identify barriers and implement culturally sensitive strategies; rotate officer-of-the-day duties and provide emergency on-call coverage. Community-based role requiring home and community visits and frequent telephone/virtual meetings.

Develop, monitor, and revise program plans; arrange services; document per regulations; attend interdisciplinary meetings; advocate with community agencies; perform outreach, on-call emergency coverage, and community-based visits.

Part-time (16 hrs/wk) student role assisting service coordinators with basic case plans, service identification/coordination, documentation, advocacy, and interdisciplinary meetings. Work includes community/home visits, virtual meetings, and culturally responsive engagement under close supervision.
