Provides school-based case management for children and families, including needs assessments, treatment planning, service coordination, documentation, advocacy, crisis intervention, trauma-informed care, and collaboration across child-service systems. The role builds community resources, supports youth and caregivers, promotes continuity of care, and maintains confidentiality and regulatory compliance. It requires working on-site in schools, participating in physical intervention training, and meeting caseload, documentation, cultural competency, and safety standards.

The role requires maintaining confidential records, participating in supervision and training, building community partnerships, and supporting youth safety, resilience, and permanency outcomes.

Provides support and services to relative and kinship caregivers and youth in the St. Cloud area. Responsibilities include outreach, community networking, connecting families with resources, facilitating support groups, maintaining communication, and completing home studies and foster-care licensing. Requires cultural responsiveness, relationship-building, a related bachelor’s degree, Minnesota driver’s license, auto insurance, and successful background screening.
