Teams at Neighborhood Family Practice

Coordinate community outreach and engagement efforts, build partnerships with community organizations, deliver culturally appropriate health education, assist patients in accessing resources, represent the organization at coalitions and events, and support program development to improve community health.

Provide comprehensive medication-related patient care in an outpatient team-based setting: dispense prescriptions, perform medication reconciliation, counsel patients, collaborate with providers, manage population health activities, prescribe within scope, supervise technicians, support PCMH model, and precept learners.

Provide primary clinical care, manage medical and psychosocial problems, supervise patient care, and participate in health committees while maintaining documentation and quality assurance.
