Provide comprehensive nursing oversight for PACE participants: assess and document health status in the EMR, plan and implement nursing interventions, administer medications, educate patients and caregivers, evaluate outcomes, coordinate with the interdisciplinary team, and advocate for participants. Care may be delivered in the PACE Day Health Center or in participant homes.

Provide outpatient internal medicine primary care within a collaborative team. Maintain a 32-hour clinical schedule plus 8 administrative hours for documentation and care coordination. Use EPIC and AI-assisted tools for charting and decision support, coordinate with specialty and on-site services, and participate in minimal on-call coverage. Eligible for loan repayment programs and clinical privileges at affiliated medical centers.

Provide triage and direct nursing care in the Emergency Department for patients of all ages, document care, administer medications/treatments, coordinate patient flow, follow up on labs/imaging, assist with discharges/transfers, provide patient education, and collaborate with the multidisciplinary team.
