Coordinate incoming and outgoing patient referrals using Epic, verify insurance and registrations, assemble clinical information, obtain prior authorizations, schedule appointments, serve as patient navigator/advocate, and maintain accurate referral documentation.

Provide direct nursing care and assessments in a clinic setting, support providers and care teams, triage patients, administer medications and wound care, coordinate referrals and prior authorizations, manage clinical phone line, document in the EMR, train support staff, and help improve patient outcomes through education and care coordination.
