Supervises care coordinators and navigators serving families with complex psychosocial needs, including trauma, abuse, and violence. Oversees referrals, follow-up, documentation, training, safety planning, and crisis support. Develops care coordination workflows and protocols, tracks outcomes, supports quality improvement, and identifies service-access gaps. Collaborates with clinical, legal, medical, research, housing, forensic, and community partners to ensure coordinated care from intake through stabilization.

Provides patient- and family-facing care coordination and navigation support across inpatient, outpatient, emergency, home, community, and telephone settings. Connects families with healthcare and community resources, supports transitions of care, conducts outreach, gathers assessment information, schedules appointments, maintains case records, and addresses social determinants of health. Collaborates with healthcare providers and community organizations while helping patients develop self-management skills.

Provides patient and family care navigation, case management support, care coordination, and community resource referrals. Conducts assessments, outreach, appointment scheduling, transitions-of-care follow-up, documentation, and patient education across inpatient, outpatient, emergency, home, and community settings. Collaborates with healthcare providers and community organizations while supporting population health programs and social determinants of health needs.
