Mountain View PACE

Mountain View PACE is a Program of All-Inclusive Care for the Elderly delivering comprehensive, personalized healthcare and social services to eligible seniors across Eastern Kentucky. An interdisciplinary team—physicians, nurses, therapists, social workers, and support staff—coordinates primary medical care, therapy, in-home services, transportation, medication and nutrition support, and adult day services so participants can remain safely and independently in their homes and communities instead of moving to nursing facilities.
