Provide case management for brain-injured patients within an interdisciplinary rehab program: review records, conduct intake interviews, chair IPPs and family conferences, coordinate services, referrals, discharge planning, document progress, communicate with families and payors, advocate for patients, and support program goals and accreditation requirements.

The Tideway Nurse Case Manager provides nursing care to clients with head trauma, supervises staff, and manages case plans ensuring client progress and advocating for their needs while coordinating with medical professionals and families.

Provide transport for residents to appointments and events, secure and care for residents while off campus, observe and document behaviors, perform Therapeutic Assistant duties, assist with ADLs and transfers, implement behavior management and emergency procedures, maintain vehicle checks and accurate records, and uphold confidentiality and organizational policies.
