Part-time Licensed Practical Nurse providing direct resident care under the RN Health and Wellness Director. Supervises CNAs, assists medication administration, conducts assessments, maintains compliance with federal/state standards, promotes resident wellness, and performs physical tasks including lifting and supporting residents.

Provide personal care, assist with and administer medications per resident care plans, monitor and document resident condition, engage residents socially, and collaborate with Wellness leadership on assessments and care documentation during the 3pm–11pm shift.

Provide direct resident care in an assisted-living community: assist with personal care and medications, monitor and document resident condition and assessments, engage residents, collaborate with Wellness leadership, and support residents with dementia.
