Teams at Mira Vista Court Rehabilitation Suites

Coordinates the Resident Assessment Instrument process, completes accurate Minimum Data Set assessments, and develops interdisciplinary plans of care. Participates in Medicare PPS interdisciplinary team meetings, collaborates with healthcare and nursing management teams, and maintains knowledge of applicable state, federal, and regulatory requirements. Requires an active LVN license, direct nursing and MDS/RAP experience, resource coordination skills, and reliable transportation.

Assists with preparing, portioning, serving, and distributing meals and snacks for patients, residents, employees, and events. Follows menus, dietary orders, food safety procedures, and patient preferences. Supports cooks with timely meal preparation and may wash dishes as needed. Requires reliable transportation, fluent English, and a high school diploma or GED; food service experience in skilled nursing or long-term care is preferred.
