Provide peer-based support to individuals with substance use challenges, perform assessments and care planning, document interactions, coordinate referrals with providers and community agencies, participate in case management and outreach, meet KPIs, and travel locally within Florida (25%–50%) for member visits.

Provide peer-based support using lived recovery experience: assess needs, develop care plans, connect members to community resources, document contacts and care plans, collaborate with care teams and community partners, attend meetings and outreach, and help meet program KPIs to support individuals affected by substance use.

Provide psychiatric evaluations, diagnose, create evidence-based treatment plans, prescribe and monitor medications, perform crisis intervention, collaborate with multidisciplinary teams, and maintain timely clinical documentation for patients in hospital settings.
