Provides nursing care in a school-based clinic, including medication administration and education, health assessments, screenings, documentation, lab-result review, prescription coordination, referrals, and communication with students, caregivers, providers, pharmacies, and medical staff. Maintains health records, supports treatment planning, completes billing documentation, and travels occasionally for training or coverage.

Maintains student health records, supports immunization follow-up, completes billing documentation, and travels up to 10% for training or coverage. Requires an Indiana-licensed, non-probationary LPN; two years of nursing experience is preferred.

Provides nutrition assessments, counseling, education, breastfeeding support, referrals, food prescriptions, and high-risk client follow-up for WIC participants. Documents services in the INWIC MIS, leads group classes, maintains client records, supports community outreach, collaborates with partner agencies, and follows Indiana WIC policies and confidentiality requirements. Travel up to 60% within Randolph, Blackford, and Jay Counties is required.
