Provide clinical assessment and care to assigned Medicare Advantage members in homes and skilled nursing facilities, identify unmet needs and gaps in care, complete summary and referral forms, plan individualized care per Matrix protocols, and initiate appropriate referrals to health plans and PCPs while working independently and using company technology.
