Provides specialist palliative nursing care across patients’ homes, community settings, and hospice. Responsibilities include complex symptom management, nurse-led outpatient clinics, clinical leadership, staff education and supervision, care planning, governance, audit, service development, and evidence-based practice. Requires registered nursing status, substantial palliative-care experience, advanced clinical skills, leadership capability, and a UK driving licence with vehicle access.

Coordinate referrals and care information for patients, families, referrers, colleagues, and clinical teams. Maintain accurate patient records, use digital systems, support rotas and scheduling, produce management and statistical reports, and resolve issues proactively. The role requires clear communication, empathy, accuracy, organisation, confidence using IT, and the ability to work calmly in a busy, multitasking environment.

Coordinate patient referrals and enquiries for a hospice, ensuring people are connected efficiently with appropriate services. Maintain accurate records across systems, schedule appointments and visits, support clinical and multidisciplinary teams, and provide compassionate assistance during sensitive conversations. The role requires strong organisation, communication, customer service, digital administration, attention to detail, resilience, and the ability to manage competing priorities in a fast-paced environment.
