The Independent Living Transition Specialist supports youth in foster care with independent living skills, assesses needs, and facilitates goal achievement through a trauma-informed approach.

Provide case management within a shelter community to develop Exit Plans, connect clients to housing, employment, benefits, and community partners; coordinate transportation and Coordinated Entry, maintain client data, perform bi-weekly reviews, collaborate with clinical team on crises, and apply evidence-based case management and shelter best practices to support housing stability.

The Administrative Support role involves managing front desk operations, answering calls, greeting visitors, and providing general administrative assistance to clients and staff.
