Coordinate clinical care for residents in transitional and long-term care, manage care plans, lead interdisciplinary teams, prevent rehospitalizations, support admissions and discharges, ensure regulatory compliance, and provide education and on-call weekend coverage.

Coordinate psychosocial services and discharge planning for skilled nursing residents. Conduct assessments, provide counseling and advocacy, connect families to community resources, assist with legal documentation, document care plans, encourage program participation, and collaborate with interdisciplinary teams to ensure comprehensive resident care.
