Coordinates the RAI and MDS assessment process for skilled nursing residents, ensuring assessments accurately reflect resident status and meet regulatory timelines. Collaborates with interdisciplinary teams on care planning and case management, verifies documentation for services and reimbursement, maintains Medicare, Medicaid, and OBRA compliance, and supports quality improvement and survey readiness.

Plans and leads recreational, social, wellness, holiday, and community programs for Assisted Living residents. Builds inclusive activities around residents’ interests, histories, and well-being, while collaborating with nursing, dining, and care teams. Encourages participation and supports an active community. The role requires a high school diploma or GED and hands-on experience planning or leading activities for older adults.

Oversee nursing care for skilled nursing and rehabilitation residents, coordinating admissions, care plans, discharge preparation, interdisciplinary collaboration, resident advocacy, documentation, and regulatory compliance. Identify condition changes, educate residents and families, support clinical and operational initiatives, and participate in weekend on-call coverage.
