Lead and manage the Social Services Department to assess residents' psychosocial needs, develop individualized plans, coordinate discharge and care planning, supervise staff, ensure regulatory compliance, maintain community partnerships, and oversee quality assurance and departmental operations.

Provides direct nursing care to residents in a skilled nursing facility: administers medications, assesses and documents conditions, coordinates care with the healthcare team, supervises nursing staff, enforces infection control and safety, and participates in QA and care planning.

Provide comprehensive nursing care to residents: assess conditions, administer medications and treatments, document and update care plans, supervise nursing staff, coordinate with physicians and Director of Nursing, ensure compliance with infection control and HIPAA, participate in QA and training, and maintain a safe environment.
