Teams at LoveWell Hospice

Provides hospice-focused nursing care to patients in homes and assisted living facilities. Responsibilities include health assessments, medication and treatment administration, individualized care planning, patient and family education, care coordination, electronic documentation, infection control, and patient advocacy. Requires an active Texas RN license, hospice experience preferred, strong clinical documentation and communication skills, and reliable transportation with a valid driver’s license and insurance.

Provide PRN hospice-focused nursing care in patient homes and assisted living: assess and monitor patients, administer medications, create individualized care plans, document in FireNote, educate families on health and bereavement, and collaborate with multidisciplinary hospice teams while adhering to infection control and safety protocols.

Provide in-home and assisted-living nursing care including assessments, medication administration, individualized care plans, patient and family education, EHR documentation (FireNote), and collaboration with multidisciplinary hospice teams. Focus on dementia and disability care, infection control, and compassionate patient advocacy.
