Teams at Longleaf Hospice LLC

Provide end-of-life hospice care in patients' homes under RN supervision. Implement and document nursing care per Plan of Care, manage pain/symptoms, educate patients/families, coordinate interdisciplinary needs, and participate in quality improvement and professional development.

The RN Case Manager provides nursing care to patients, coordinates with the Interdisciplinary Team, conducts assessments, and manages treatment plans, ensuring holistic patient support.

Provide hospice nursing care in patients' homes: assess needs, develop and modify care plans, manage symptoms, triage calls, educate families, document and coordinate with interdisciplinary team.
