Teams at Longleaf Hospice LLC

Provide end-of-life hospice care in patients' homes under RN supervision. Implement and document nursing care per Plan of Care, manage pain/symptoms, educate patients/families, coordinate interdisciplinary needs, and participate in quality improvement and professional development.

The RN Case Manager provides nursing care to hospice patients, conducts assessments, manages treatment plans, and coordinates with the health care team.
