Provide nursing care and case management for hospice patients in-home, including assessments, symptom management, care planning, patient/family education, triage, documentation, and coordination with the interdisciplinary team.

The Float Nurse provides nursing care management, triages calls, modifies treatment plans, and documents patient conditions, working across multiple Longleaf offices.

The RN Case Manager provides nursing care to hospice patients, conducts assessments, manages treatment plans, and coordinates with the health care team.
