Teams at LOFT Community Services

Provide behaviour support assessments and person-centered behavioural support plans for older adults with dementia, mental health or neurological conditions; coordinate transitions from hospital to community, deliver direct care, crisis intervention, training and consultation for families and care teams, maintain records, liaise with multidisciplinary partners, and support program planning and meetings.

Provide intensive case management and wraparound supports for clients with mental health, substance use, and homelessness. Develop recovery plans, coordinate community and medical supports, provide crisis intervention and on-call coverage, assist with daily living skills, tenant mediations, recordkeeping, and preparing units for occupancy.

Lead cultivation and management of corporate and foundation donors, focusing on six-figure gifts. Oversee grant research, proposal development, submission, reporting, stewardship, pipeline management, and donor relations. Manage a small team, budgets, and fundraising metrics while collaborating with stakeholders to align proposals with organizational priorities and ensure compliance with funder requirements.
