Teams at LifeLong Medical Care

Coordinates front office operations at a community health center, including patient reception, registration, scheduling, check-in, eligibility screening, phone support, referrals, cash collection, records, correspondence, supplies, and patient-flow management. Maintains eligibility information, supports provider templates, tracks statistics, and collaborates with clinical staff. Requires strong customer service, organization, confidentiality, Microsoft Office proficiency, medical-field experience, and bilingual English/Spanish skills.

Provides care coordination and case management for patients with complex medical, behavioral health, and social needs. Conducts outreach, assessments, care planning, referrals, health education, benefits and housing assistance, crisis intervention, advocacy, and documentation. Works across homes, clinics, hospitals, shelters, and community settings while collaborating with patients, caregivers, care teams, and external partners.

Provides case management for patients with complex medical, behavioral health, and social needs. Maintains patient caseloads, documents services in EHR and HMIS systems, coordinates interdisciplinary care, and supports underserved populations through home, clinic, and community-based services.
