Teams at Lifeline Connections

Provides community-based behavioral health case management and vocational services for patients with serious mental illness and substance use conditions. Duties include individualized support planning, activities of daily living assistance, housing and benefits support, social-skills coaching, transportation coordination, clinical documentation, and collaboration with multidisciplinary teams. Vocational responsibilities include IPS-supported employment, job development, vocational assessments, job coaching, benefits counseling, resume preparation, interview support, and employer coordination.

Provides clinical mental health services, including individual and group therapy, case management, diagnostic assessments, treatment planning, behavioral support, and skills training. Maintains compliant clinical records, participates in treatment teams, coordinates continuity of care, and represents the agency in community settings. Requires collaboration with patients, families, community providers, and legal or healthcare representatives while supporting recovery, resilience, dignity, and patient choice.

Provides community-based behavioral health case management and re-entry support. Conducts assessments, develops individualized care plans, coordinates behavioral health, medical, housing, transportation, employment, benefits, and other services, and facilitates referrals. Collaborates with correctional facilities, providers, courts, probation, managed care organizations, and community agencies. Maintains compliant documentation, advocates for participants, supports recovery-oriented and trauma-informed care, and participates in supervision, training, quality improvement, and community engagement.
