LIFE Pittsburgh

LIFE Pittsburgh is a nonprofit program that provides older adults who need nursing-home-level care with an alternative that helps them remain safely and independently in their homes and communities. Through the PACE model, it coordinates comprehensive medical, social, and daily-living services, including primary care, nursing, therapy, medications, transportation, and in-home support. Interdisciplinary teams tailor and oversee care through its day health centers and home-care program.
