Teams at Konza Prairie Community Health Center

Provides outpatient primary care support in an FQHC, including patient assessments, triage, medical history documentation, medication and immunization administration, treatments, procedures, referrals, patient education, phone triage, and accurate EMR maintenance. Collaborates with providers and integrated behavioral health teams to deliver compassionate, evidence-based care to patients of all ages.
