Provides strengths-based, trauma-informed case management and counseling to youth in a behavioral rehabilitation program. Responsibilities include delivering treatment programming, maintaining case files and service plans, supporting admissions and aftercare, documenting progress, transporting clients, and collaborating with treatment professionals, probation officers, and families.

Provides administrative and clerical support to a developmental disabilities eligibility team and department leadership. Duties include documentation review, data entry, filing, chart maintenance, report and spreadsheet preparation, eligibility timeline tracking, record-of-release processing, caller assistance, and coordination of on-site testing. The role requires maintaining confidential health information, following HIPAA and department procedures, meeting deadlines, and supporting clients, families, providers, and county staff.

Provides targeted case management for individuals with intellectual and developmental disabilities. Develops and monitors individualized and annual support plans, coordinates services with families and public and private agencies, authorizes services, maintains a caseload, documents progress, supports transitions, and ensures compliance with Oregon regulations, Medicaid standards, confidentiality requirements, and mandatory reporting obligations. Requires Spanish fluency, strong communication skills, human services knowledge, organization, and a valid Oregon driver’s license.
