Coordinates case management for inpatient, observation, emergency, swing-bed, and resident-home patients. Assesses admission-related discharge needs, develops discharge plans, arranges home services and durable medical equipment, coordinates short- and long-term placement, and assists with financial, emergency, and psychiatric placement needs. Serves as a patient advocate while applying third-party payer guidelines and inpatient criteria.

Coordinates educational programs for hospital and resident home employees, develops policies and education plans, leads department-specific training, and ensures compliance with nursing practice standards. The role partners with hospital leadership to support safe, high-quality patient care through effective training modules, presentations, communication, and collaborative education initiatives.

Full-time pharmacist providing care across inpatient, outpatient, and nursing home settings within an integrated hospital system. Responsibilities include supporting pharmacy services, communicating effectively across departments, and helping develop clinical programs. The role works four days per week, includes weekend rotation, and has no overnight shifts.
