Registered Nurse Case Manager coordinates and manages patient care: conducts assessments, develops care and discharge plans, coordinates with multidisciplinary teams and community resources, provides patient education, documents progress, and monitors outcomes to ensure continuity of care.

Provide direct nursing care in the Cardiology Department including assessing patient needs, performing diagnostic cardiology and nuclear medicine procedures (pharmacologic stress testing, conscious sedation, TEE, echo contrast), and maintaining the pacemaker clinic. Maintain nursing standards, follow policies/procedures, and communicate effectively while ensuring confidentiality.

The Telemetry Monitor Technician monitors cardiac rhythms of patients, reports arrhythmias, assists healthcare staff, and participates in patient care improvement initiatives.
