Provide outreach and case management: assess client needs, create and implement service plans, refer to housing/food/benefits, maintain confidential records in E-Compass and CAREWare, prepare reports, represent agency at community events, and coordinate with social services and insurers.

Provide community-based harm reduction case management: conduct outreach and assessments, connect clients to housing, transportation, employment and psychosocial supports, maintain client records and reporting, represent the agency at community events, and participate in required trainings. Work includes follow-up on referrals and occasional nights/weekends.

Provide harm-reduction focused community case management in Hudson County: conduct outreach and education, recruit and assess clients, develop service plans, make referrals to medical and substance-use treatment, maintain confidential records and reports, represent Hyacinth at community events, and attend trainings. Work may include nights and weekends.
