Provides home hemodialysis treatment support to patients within the community under the direction of a registered nurse. Responsibilities include monitoring patients and machine alarms, documenting treatment and status changes, handling supplies and equipment, following infection-control and safety protocols, reporting concerns, and participating in training and quality-improvement activities. The role requires travel within the hospital’s catchment area and rotating shifts, including evenings and weekends.

Performs non-invasive cardiac diagnostics including stress testing, ECGs, Holter monitoring, ambulatory blood pressure monitoring, spirometry, and cardiac implanted electronic device testing. Supports device implantation, troubleshooting, programming, emergency response, patient education, documentation, and collaboration with physicians. Works within hospital policies while delivering safe, accurate, patient-centered care.

Provides physiotherapy assessment and evidence-based treatment for post-acute and acute-care patients. Manages caseloads, documents assessments and discharge plans, supports interdisciplinary care, supervises Rehabilitation Assistants, and teaches physiotherapy students. The role includes cross-coverage across hospital and reactivation care sites, workload measurement, and weekend rotation responsibilities.
