Teams at Hospital Sisters Health System

Performs radiographic imaging for patients of all ages in the hospital’s X-ray department. Operates imaging equipment and systems, works independently across weekend day shifts, and maintains patient-focused care. Requires an accredited radiologic technology education, applicable state accreditation or licensure, ARRT radiography certification or eligibility, and Basic Life Support certification.

Coordinates patient care and discharge planning across patients, families, clinicians, social workers, and payers. Assesses patient needs, manages transitions and throughput, promotes safe and cost-effective care, monitors appropriate care settings, supports regulatory compliance, and evaluates outcomes. The role requires an Illinois or Wisconsin RN license and three years of clinical experience, with case management and certification preferred.

Coordinates patient care and discharge planning across interdisciplinary teams, payers, patients, and families. Assesses patient needs, facilitates safe transitions, manages throughput and length of stay, supports cost-effective care delivery, and ensures compliance with regulatory and organizational standards. The role requires an RN license, clinical experience, and collaboration across the care continuum.
