Provides compassionate, patient-centered case management for home health or hospice patients and families. Coordinates care plans, builds patient relationships, communicates in English and Spanish, and works autonomously in the field while collaborating with an experienced care team. The role requires an active Colorado nursing license, bilingual communication skills, strong organization, and a commitment to comfort, dignity, and exceptional care.
