Coordinates hospice patient care, staff visit scheduling, caseload monitoring, chart audits, and clinical performance tracking. Supports patients, families, clinicians, and clinical managers while ensuring regulatory scheduling requirements, quality standards, HIPAA compliance, and organizational procedures are followed. The role requires an LPN or therapy assistant licensed in Rhode Island and Massachusetts, with at least two years of clinical experience.

Coordinates community services programs, volunteers, events, outreach, registrations, communications, data management, mailings, and administrative processes. Provides customer service, represents HopeHealth at community events, supports program materials and vendor coordination, and communicates the value of hospice, palliative care, grief support, caregiver, and other community services. Maintains accurate records, schedules, invoices, payments, and program information while collaborating with internal teams and external partners.
