Provide clinical case management using nursing experience to assess patient needs, develop care plans, coordinate services, and advocate for patients. Work weekdays in-office (hybrid after orientation). Specialty tracks available (Oncology, Chronic Condition, Behavioral Health). Minimal physical exertion; occasional onsite patient evaluations required.
