Provide supervision and bi-weekly supervision to case managers and interns while managing a tenant caseload (max 15). Conduct assessments, create individualized service plans, provide counseling, benefits coordination, service linkages, referrals, group facilitation, and housing stability supports. Audit charts, liaise with medical/behavioral providers, coordinate employment/education linkages, support rent timeliness, collaborate with property management, document using AWARDS and maintain required reports and records.

Supervise three Case Managers while managing a caseload of 23 clients. Perform assessments, counseling, referrals, chart reviews, and quality assurance. Ensure documentation and data entry in the AWARDS system, run weekly case conferences, coordinate a Money Management Program, and provide individual and group counseling to help clients obtain permanent housing.
