The role collaborates with clinical and care management teams and requires compliance with pharmacy regulations and protected health information standards.

Coordinates clinical quality improvement activities for a kidney health management program. Partners with providers and care navigation teams to address care gaps, improve HEDIS performance, educate practices on chronic kidney disease, support workflow improvements, identify members needing care navigation, and maintain NCQA-compliant documentation. Requires regular travel to healthcare practices across New York City and surrounding assigned markets.
