Provides skilled home health nursing and case management for patients recovering at home. Conducts admissions and assessments, develops care plans, administers physician-ordered treatments, monitors patient conditions, educates patients and caregivers, and supervises LPNs and home health aides. Maintains documentation, coordinates care, meets regulatory and payer requirements, manages caseload productivity, and evaluates patient outcomes. The role includes a 12-week nurse residency orientation with classroom instruction and supervised field experience.

Oversees clinical operations for a home health agency, supervising interdisciplinary staff and ensuring high-quality, compliant patient care. Responsibilities include clinical supervision, care-plan oversight, staffing, scheduling, performance management, recruitment, training, quality improvement, case conferences, budgeting, compliance, and administrative on-call coverage. The manager also conducts in-home supervisory visits, supports policy development, and drives team performance and organizational goals.

Coordinates high-volume employee onboarding from pre-employment requirements through first-day file handoff. Manages background checks, drug screens, compliance documentation, vendor communications, orientation logistics, onboarding records, and new-hire communications. Maintains HR registries and checklists, supports process improvements, assists with employer branding events, and provides general HR project and office support while ensuring compliance with company policies and employment regulations.
