Teams at HCR Home Care

Provides skilled nursing care to home health patients while supervising, evaluating, and mentoring nurses and other clinical employees. Conducts in-home orientation, co-visits, classroom training, competency evaluations, supervisory visits, case conferences, and quality initiatives. Maintains regulatory and payer guideline compliance, documentation, clinical competency, and multidisciplinary collaboration. Requires an active New York RN license, home care experience, CPR certification, and a valid New York driver's license.

Monitors and audits home health clinical records, OASIS documentation, hospitalizations, and quality measures for regulatory compliance and improved outcomes. Analyzes performance trends, identifies gaps, educates clinicians on documentation standards, supports QAPI initiatives, and collaborates with clinical leadership and interdisciplinary teams. Requires occasional travel to branch offices or field meetings.

The Assistant Controller manages day-to-day accounting for multiple legal entities, month-end close, journal entries, intercompany transactions, financial statements, internal controls, compliance, audits, and cost reporting. The role supports budgeting, financial analysis, profitability reviews, and executive reporting while communicating financial information to stakeholders and non-financial teams. A bachelor's degree and at least five years of professional accounting experience are required; CPA certification is preferred.
