Coordinates interdisciplinary resident care planning and leads IDT meetings in a long-term care setting. Completes accurate and timely MDS assessments, selects assessment reference dates and diagnosis codes, supports PDPM classification and reimbursement, validates documentation, monitors quality measures, and ensures CMS and regulatory compliance. The role also oversees nursing care delivery and promotes resident dignity, safety, and quality of life.

Provides hands-on meal service in a skilled nursing facility, including preparing snacks and special feedings, setting up trays and dining tables, verifying meal accuracy, serving residents, clearing dishes, handling linens, rotating stock, and cleaning and sanitizing work areas. Works cooperatively with residents, families, guests, management, and culinary staff while following dietary orders and facility procedures.

Prepares therapeutic desserts and salads, serves meals to residents, sets up and breaks down dining areas, transports food and supplies, maintains cleanliness and safety, follows hygiene standards, and attends training. The role requires clear communication, basic math, reliable attendance, and the ability to follow instructions. Food service preparation experience is required, while senior living and healthcare experience are preferred.
