Teams at GuideStar Eldercare

Serve as primary liaison between contracted facilities, clinicians, scheduling teams, and patients. Manage and expedite referrals in the EMR (Gehrimed), maintain accurate patient records and registrations, track documentation, report trends to leadership, and provide excellent customer service to patients, families, staff, physicians, and vendors.

Support clinical operations by troubleshooting IT issues, managing asset onboarding/offboarding, coordinating facility onboarding communications, assisting with EMR support and reporting, tracking vendor invoices and QA reviews, creating process documentation, analyzing operational data, and collaborating on projects with leadership and operations teams.
