Coordinates and troubleshoots daily and monthly clinician schedules across contracted facilities. Serves as a liaison between clinicians, facility staff, and internal teams; monitors referrals and patient volumes, identifies coverage needs, updates contact records, handles facility questions, reports critical issues, and assists with patient and facility data entry.

Provide neurobehavioral health services to elderly residents in long-term care facilities. Conduct assessments and deliver holistic, continuous care for individuals with neurocognitive, neurologic, and psychiatric conditions while collaborating with interdisciplinary specialty clinicians. Maintain documentation in an EMR, work autonomously with regional support, and contribute to treatment planning for geriatric patients.
